Wheel fell off 🥴

Driving back in the 550 from our daughters in Scotland today and had the great thrill of the near side rear wheel making a bid for freedom, thankfully at slow speed.
The car has just had a major service at a very well respected indi just a couple of weeks ago and has travelled around 500-600 miles since.
Initially there was a bit of a vibration and a noise which I thought may be part of the undertray coming loose . Inspection showed nothing but whatever was making the noise got progressively worse and seem to be speed dependent. Another stop, a good look around but nothing to be seen. Decided to keep the pace to below 40mph but with a very sudden loud crunch the rear wheel came off. I found 4 of the 5 nuts. 4 were ok but 1 has slight thread damage.
Damage to the car seems minimal - scratched calliper, flattened brake line but the wheel is damaged where it bolts onto the hub. Not sure if this can be skimmed or a new wheel reqd? Also concerned that the hub itself may have thread damage.

If it were mine I’d definitely replace that wheel. It’s the damage on the inner surface that would worry me more than the flange damage. It’s the last thing you want to fail on a car. Especially with how much torque goes through them.
The hub might be salvageable depending on how damaged any or all the threads are but I’d probably replace that as well for piece of mind along with the wheel bolts.
 
I think I'd be taking it up with the indie who did the major. I'm not sure how that could have happened if that wheel was properly torqued up. I know at the main dealer near me when the wheels go back on a different mechanic has to come over and check they are torqued up, have seen them do it.. I realise advice is to retorque wheels having driven the car for a few miles but how many people actually do this? I'm guessing most people out there don't own a torque wrench and it's not as if wheels are falling off cars all the time. Probably a stupid question but have you checked all the other wheels are torqued to the correct number?
 
You should periodically re torque them anyhow .
Not being critical but this happens a lot .
It was never done basically at the last time it was removed . I trust you have checked the other 3 ?

As you say the wheel is scrap now .
